There are four types of incident forms based on the selected claimant type: Customer, Team Member, Property, or Other. All forms have the same fields appearing in the “General Information,” “Witnesses,” and “Supporting Documents” sections, but they each have a different set of fields appearing under their “Claimant Information” section. The tables below detail the fields that appear in each section of the incident form (fields that are mandatory have a blue background). The “Claimant Information Section” table has an additional column to specify which forms each field appears on.
